The form on this page should be used to report on the performance of game referees. Currently, this is not a requirement, however, it is an opportunity for coaches to provide candid feedback on the performance of the game officials that were assigned to work their games. League officials will review all feedback and may follow-up with the reporting individual if additional information is needed.
